Thank you for considering Shelton Veterinary Care for your pet’s needs. It is our commitment to provide quality veterinary care throughout the life of your pet.
Prior to scheduling a new appointment, we must have the following:
- A completed Registration form for every animal. This provides us with the best information for you and emergency contacts, gives us important information regarding your pet(s), and also indicates your understanding of financial responsibility for appointments. Please call or text with your name, your email address and reason to be seen and we will email you this form.
- A copy of every animal(s) prior medical records that you are wishing to schedule. To provide the best clinical care possible, it is imperative to have complete medical history as well as vaccine records. This helps to provide history of treatments given before, known medical conditions, previous and current medications, prevention methods, and other important information needed for the continued health and safety of your pet and staff members providing care for them.
There are several ways to provide a copy of your pet(s) medical records. Your previous provider may have already provided you with a copy of their records or you can contact them to have the previous provider send them to us directly. Records can be sent to us via email at sheltonvetcare@gmail.com. Records can also be faxed to 317-392-7946.
Once we have received BOTH of the items listed above, please allow at least 48 hours for a member of our clinic to contact you and an appointment will be scheduled at this time.
Please note:
- For the safety of our community and staff, Shelton Veterinary Care requires the rabies vaccine for all pets. Indiana law requires the that dogs and cats be up to date on the Rabies vaccine. If not current, we will update at the first appointment.
- Cats MUST be in a carrier and dogs MUST be on a leash. NO EXCEPTIONS! Please understand this is a serious safety issue.
- Payment is due at the time of the appointment.
- You must cancel or reschedule 24 hours before the appointment or there may be a fee.
